What to bring
A short checklist to help your first visit go smoothly.
Previous reports
Any medical reports, test results, X-rays, or discharge summaries from other clinics or hospitals.
Current medicines
A list of all medicines being taken — or the actual boxes/packets. Include dose and frequency if known.
Vaccination card
For children, bring the vaccination card. It helps us review milestones and avoid duplicate doses.
ID for registration
Bring your CNIC or B-Form for the patient. First-time visitors need to register with us.
List of questions
Write down any questions or concerns you want to discuss. It's easy to forget during the visit.
Arrive 10 minutes early
This gives us time to register you, take vitals, and settle in before your appointment begins.
